Athletic Forms and Payments
Please click the link above to download the sports physical packet. All physicals must be submitted prior to any sport participation.
Students playing a sport are required to pay an athletic fee of $75 per sport. Athletic fees must be paid prior to the first season practice. This fee pays for athletic equipment and supplies.
